HFA Supervision Forms Overview and Guidance
These tables provide a description of the documentation required to complete the self study as
The HFA Best Practice Standards (BPS) describe expectations for fidelity to the Healthy Families America model. Herein referred to as the Standards, they are structured around the twelve research-based critical elements upon which the Healthy Families America (HFA) model was designed. The critical elements serve as the overarching ‘big ideas’ defining the Healthy Families America model. The Standards also include a section on Governance and Administration which articulates expectations for effective site management.
For a high level overview of structural changes within the BPS – standards which changed in number, were moved, were added, or deleted, this at a glance document puts the table of contents side-by-side so you can see those specifics.

Considerations for dual role PM/Sup, incl. supervisor ratios
Not all HFA sites use the title “Program Manager” for staff in this role, and very small sites may choose to combine the role of the Program Manager as defined here with a Supervisor role, providing support to direct service staff.
Best Practice Standard 12-1.D defines the ratio of staff to Supervisors at an HFA site, to ensure effective supervision for direct service staff working with families. A full time (40 hours/week) Supervisor can supervise up to 6 direct service staff, with a recommendation that sites limit the ratio to 5 staff for a full time supervisor. Best Practice Standard 12-4 describes the way a site creates supports and holds Program Managers accountable for their work. The Tip included in that Standard state7/30/21: Resource coming soon!
